Nominations must include name, mailing address and telephone number for the nominee and for the contact person in your organization. Use the enclosed application form and any other paper necessary to help the selection committee ascertain the civic contributions of your candidate.
Completed applications must be received no later than Friday, December 29, 2017.
First Citizen Award:
Selection will be based on the nominee’s civic involvement & his/her contribution to the welfare and improvement of Florence and/or the surrounding communities. The specific elements to be considered by the selection committee are:
1. Length of service
2. Current involvement
3. Current leadership activities included in #2
4. Past service
5. Leadership in the activities included in #4
6. Impact on the overall community
Each of the above items will be rated by the selection committee on a scale of 1-10. The attached questionnaire must be complete and specific in every category in order for the nominees to receive proper credit for his/her service. Please remember that only individuals can be nominated for this award.
